Certified Medical Assistant- Chronic Care Management

In this role, you'll help deliver high-quality, member-centered care and play a key part in improving clinical outcomes, supporting care coordination, and enhancing the experience for both members and their caregivers. Responsibilities Review Community Health Worker (CHW) appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers Mail and/or email patient care plans and educational materials as directed by case managers Accurately update and maintain patient records within the electronic health record (EHR) system Protect patient confidentiality and support compliance with HIPAA and organizational policies Requirements Certified or Registered Medical Assistant (CMA/RMA) required 2 years of experience in a medical office, telehealth, or care coordination setting Experience using EHR systems; experience using eClinicalWorks preferred Experience working within HIPAA and organizational policy requirements in a healthcare setting Available during Eastern Time business hours Experience working with older adult populations preferred Benefits Health, Dental, and Vision Insurance Benefits 401k PTO, Sick, Wellness leave, and Paid Holidays #J-18808-Ljbffr Belle
