Key Responsibilities\n

  • Conduct comprehensive assessments of patients' psychosocial needs to develop tailored treatment plans that address their unique situations.

  • Provide case management, including short-term counseling and crisis intervention, to support patients during critical times.

  • Guide patients through the process of obtaining necessary entitlements, insurance benefits, and financial assistance, reducing barriers to care.

  • Assist patients in accessing prescription cost assistance through various indigent programs and available community resources.

  • Advocate for patients by effectively communicating with community agencies to facilitate support services and resources.

  • Maintain accurate and comprehensive patient records, ensuring all documentation meets legal and ethical standards, while preparing detailed reports as necessary.

  • Coordinate transportation assistance for clients when required to ensure their access to services.

  • Participate proactively in clinical meetings, including case conferences, to collaborate with peers and contribute to holistic patient care discussions.

Patient Population / Client Focus\n

The primary patient population includes individuals experiencing a wide variety of mental health challenges, including anxiety disorders, depression, trauma-related issues, and other psychological conditions. Qualifications\n

  • A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling from an accredited institution is required.

Compensation and Benefits\n

The salary for this position ranges from USD 120,000 to 130,000 annually, commensurate with experience.
