Job Overview Adult Community Clinical Services (ACC) is a Department of Mental Health funded service delivering evidence-based interventions within a clinically focused model. Clients are diagnosed with mental illness or dual diagnosis and reside in homes, apartments, or group living environments. As a Relief Counselor, you help individuals transition to more independent environments, maximize natural supports, and build skills to achieve personal goals. Responsibilities Assist in developing individualized, person-centered treatment plans and lead daily implementation plans. Assist individuals to access and/or provide transportation. Implement activities that create opportunities for the development of valued roles and personal relationships in the community. Review and complete all required documentation. Provide direct support care, ensuring daily living needs are met while maintaining a safe yet inspiring atmosphere. Qualifications High School Diploma, GED or equivalent, required. Valid Driver’s License and acceptable driving record, required. Reliability, willingness to learn, and openness to new opportunities. Additional hourly differential may be offered based on skills, location, training, and population served. Discuss with recruiter.
