Impactful Health Educator & Case Navigator

accesscommunity seeks a Case Worker – Health Educator to provide education and advocacy within the Community Health and Research Center. You will coordinate care, navigate systems, and teach clients how to manage their health needs. The role requires an Associate degree and 2+ years in relevant fields; experience with EMR systems and Microsoft Office is preferred.
