We are proud to be a part of the UMMS network.Under general supervision, the Community Health Worker (CHW) is a frontline public health worker who performs a wide variety of duties at the paraprofessional level to engage, support, advocate and educate Southern Maryland community members in overall health and wellness. The CHW serves as a liaison or link between health and social services and the Charles County community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.Skills and abilities:Knowledge of local population demographics, assets and needsKnowledge of local community health disparities such as diabetes and hypertensionAbility to connect with and provide ongoing support in a behavior change processDemonstrated organizational and time management skillsEffectively communicate in writing in order to document patient information and progressAbility to follow instructions and incorporate feedbackCommunicate verbally with patients and co-workers in a clear and effective mannerAble to work independently and as part of a multidisciplinary teamKnowledge and skills to screen high risk patients for post discharge needs and ensure the effective communication is established with the patients, caregivers, families, and all disciplines to ensure a successful transition across the continuum of careThe ability to ensure the confidentiality and rights of all patient and departmental documents is requiredBenefits:All your information will be kept confidential according to EEO guidelines.Compensation:Pay Range: $18.57-$25.99
