This is an in-person home health position that offers flexibility in daily scheduling, along with manageable caseloads and meaningful one-on-one patient care.Qualifications\n \n Active Registered Nurse (RN) License in the State of Colorado (or active NLC compact license)\n Valid driver’s license, reliable transportation, and auto insurance\n Strong assessment and critical thinking skills\n Ability to work independently and manage a structured caseload\n Clear, professional communication with patients and team members\n Comfort with documentation and EMR systems (HCHB preferred)\n Home health experience preferred, but not required\n Responsibilities\n \n Perform comprehensive patient assessments\n Develop and update individualized plans of care\n Provide skilled nursing care and appropriate interventions\n Monitor patient progress and report changes to the care team\n Educate patients and families on medications, treatments, and care plans\n Coordinate care with LPNs, therapists, caregivers, and providers\n Complete accurate and timely documentation in EMR\n Participate in on-call rotation as needed\n Work Environment\n Home health offers a different pace and structure than facility-based care.
