We are seeking a licensed and experienced Nurse Practitioner (NP) to provide high-quality primary care services one day per week. The Nurse Practitioner will independently evaluate, diagnose, treat, educate, and manage patients in accordance with Louisiana state regulations and clinic protocols. The NP will be responsible for comprehensive patient care, accurate documentation, medication management, chronic disease monitoring, preventative care, and collaboration with the supervising physician when appropriate.

Essential Duties and Responsibilities

Patient Assessment and Treatment

• Perform comprehensive medical evaluations and physical examinations.
• Obtain and document patient histories.
• Diagnose and treat acute and chronic medical conditions.
• Develop individualized treatment plans.
• Order, review, and interpret laboratory tests, diagnostic studies, and imaging results.
• Provide preventative care and wellness services.
• Conduct annual wellness visits and routine follow-up appointments.
• Assess patient risk factors and provide counseling on disease prevention.

Medication Management

• Prescribe medications within the NP's scope of practice and Louisiana regulations.
• Refill medications after appropriate review of patient records and clinical necessity.
• Monitor medication effectiveness and adverse reactions.
• Adjust medication regimens as clinically indicated.
• Review controlled substance usage according to clinic policies and state regulations.
• Complete prior authorizations when required.
• Provide medication education and counseling to patients.

Chronic Disease Management

Manage patients with conditions including but not limited to:

• Hypertension
• Diabetes Mellitus
• Hyperlipidemia
• Obesity
• Thyroid Disorders
• COPD
• Asthma
• Heart Disease
• Depression and Anxiety (within scope and clinic protocols)

Preventative and Wellness Care

• Annual physical examinations.
• Medicare Annual Wellness Visits.
• Preventative screenings.
• Vaccination recommendations and administration oversight.
• Lifestyle counseling and patient education.

Documentation and Charting

• Complete all patient charting within 24 hours of the encounter.
• Document assessments, diagnoses, treatment plans, prescriptions, and patient communications accurately in the Electronic Health Record (EHR).
• Maintain compliance with HIPAA, CMS, Medicare, Medicaid, and commercial payer documentation requirements.
• Ensure charts support medical necessity and billing compliance.
• Respond to chart audits and documentation reviews as requested.

Care Coordination

• Coordinate care with physicians, specialists, hospitals, home health agencies, and ancillary providers.
• Review consultation reports and diagnostic findings.
• Communicate treatment plans and follow-up recommendations to patients.
• Participate in quality improvement initiatives when requested.

Administrative Responsibilities

• Review and respond to patient portal messages during contracted clinic hours.
• Review laboratory and diagnostic results and communicate findings to patients.
• Complete medical forms and documentation related to patient care.
• Participate in provider meetings as requested.
• Maintain current licensure and certifications.

Scope of Practice

The Nurse Practitioner shall:

• Practice within Louisiana Nurse Practice Act guidelines.
• Maintain a collaborative practice agreement as required by state law.
• Exercise independent clinical judgment within the scope of licensure and training.
• Refer patients to higher levels of care or specialists when clinically indicated.
• Follow all clinic policies, procedures, and standards of care.

Qualifications

Required

• Current Louisiana Nurse Practitioner License.
• National Board Certification (AANP or ANCC).
• Current DEA Registration.
• Current BLS Certification.
• Minimum two (2) years of primary care experience preferred.
• Experience with Electronic Health Records (EHR).
• Strong knowledge of Medicare, Medicaid, and commercial payer documentation requirements.

Preferred

• Family Nurse Practitioner (FNP-C or FNP-BC).
• Experience in rural health or primary care settings.
• Knowledge of value-based care initiatives.
• Experience managing chronic disease populations.

Compensation

• Competitive daily contract rate.
• Independent Contractor (1099) status.
• No employee benefits provided.
• Professional liability insurance required and must be maintained by the contractor unless otherwise agreed upon.
