Clinical Pharmacist - Part-time

About the Role The Clinical Pharmacist works in conjunction with a multi-disciplinary team to provide safe, high quality infusion therapy to patients in the alternate site. This role is responsible for evaluating and monitoring drug therapy relative to the total needs/problems of the patient while collaborating with physicians, nurses, and other healthcare professionals in an effort to achieve the best possible outcomes. Schedule: Monday – Friday; 8:00 AM – 5:00 PM; 3 days a week Minimum Qualifications Experience/Education: Bachelor of Pharmacy Clinical Pharmacy experience from work or clinical rotation experience Proficient at partner programs (i.e.: MS Outlook, Word, Excel). Ability to cope with ambiguous, changing environment while under pressure in a calm Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team. Essential Functions Reviews, receives, and verifies all patient orders and prescriptions and documents according to policy and procedure and state and federal pharmacy law Participates in the direct supervision of Pharmacy Technicians, Pharmacy Support Technicians, and Pharmacy Logistics Technicians Participates in on call rotation for after-hours availability for either home infusion or ambulatory infusion Maintains proper competencies based on assigned role Maintains active licensure and current on all required continuing education Acts as a drug information source for internal staff and outside referral sources Provides comprehensive patient counseling to patients who accept and request a pharmacy consult Reviews clinical information including applicable labs and patient information for the safe provision of medication Adheres to all federal and state pharmacy law and regulations Clarifies and receives orders from a physician or medical agent Participate in weekend/weekday on call schedule at the discretion of the pharmacy manager/pharmacist in charge.
