Responsibilities
- Administer prescribed medications and document administration accurately in the MAR.
- Monitor vital signs, assess patient status, and report changes to the supervising RN.
- Perform routine nursing tasks including ADLs, wound care, dressing changes, and injections as ordered.
- Maintain up-to-date care plans and thorough charting; participate in care rounds and interdisciplinary communication.
- Ensure patient safety, implement fall precautions, and respond promptly to call bells.
- Collaborate with the interdisciplinary team and coordinate with physicians, therapists, and family as needed.

Requirements
- Active LPN license in North Carolina (in good standing)
- Graduation from an accredited practical nursing program
- Long-term care experience preferred
- Ability to work 7:00 PM-7:30 AM night shifts
- Strong communication and documentation skills; reliable team player
- Eligible for travel assignment and able to adapt to a nursing home LTC setting
