Responsibilities

• Administer medications and treatments within the scope of practice and facility policy
• Monitor residents’ condition, document vital signs, intake/output, and report changes to the supervising RN
• Assist with activities of daily living and provide basic nursing care for residents
• Collaborate with the Registered Nurse, interdisciplinary team, and care plans to ensure safe, individualized care
• Maintain accurate, timely documentation and uphold infection prevention and safety standards
• Respond to resident needs and call lights promptly, ensuring resident safety during night shifts

Requirements

• Active Licensed Practical Nurse license in New Hampshire or eligibility to practice in NH
• Current Basic Life Support (BLS) certification or ability to obtain
• Graduation from an approved practical nursing program
• Minimum of 1 year of licensed practical nursing experience in long-term care or skilled nursing setting (preferred)
• Ability to work night shifts (7:00 PM–7:00 AM) and participate in a travel assignment
• Access to appropriate means to comply with background check and drug screening as part of the employment process
