Compensation: $98,000-$108,000 annually (DOE)

Position Overview
Wheeler Staffing Partners is seeking a compassionate Physical Therapist, Home Health to provide skilled, patient-centered care throughout the Loveland, CO area. This field-based role is responsible for evaluating patients, developing individualized treatment plans, improving mobility and function, and collaborating with an interdisciplinary team to deliver exceptional home health services.
Key Responsibilities

• Provide skilled physical therapy services to patients in their homes throughout the assigned coverage area.
• Conduct comprehensive patient evaluations to assess mobility, strength, functional limitations, and rehabilitation needs.
• Develop and implement individualized treatment plans based on patient assessments and physician orders.
• Deliver therapeutic exercises and interventions to improve mobility, balance, strength, and independence.
• Monitor patient progress and adjust treatment plans as needed to achieve established goals.
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care.
• Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safe rehabilitation practices.
• Maintain accurate and timely documentation of patient visits, treatment plans, and clinical progress.
• Ensure compliance with applicable healthcare regulations, clinical standards, and organizational policies.
• Participate in required on-call coverage and travel to patient homes throughout the assigned service area.

Required Qualifications

• Master's degree in Physical Therapy from a Commission on Accreditation in Physical Therapy Education (CAPTE)-accredited program.
• Minimum of 9 months of professional experience as a Physical Therapist.
• Current, active Physical Therapist license in Colorado.
• Valid CPR certification, maintained throughout employment.
• Valid driver's license and reliable transportation for travel to patient locations.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication, organizational, and patient education abilities.
• Ability to work independently in a home-based care environment while collaborating with an interdisciplinary team.
• Must successfully complete a background check and drug screening.

Preferred Qualifications

• Previous experience in home health, community-based care, or a similar rehabilitation setting.
• Experience managing patients with chronic conditions, mobility limitations, and post-acute rehabilitation needs.
• Familiarity with home health documentation and care coordination processes.
