At Select Home Health & Hospice, we believe in putting patients first: “Select the best for your health!” Job Summary Select Home Health is seeking a skilled and compassionate Registered Nurse (RN) – Float to provide high‑quality, in‑home nursing care. This role supports multiple patients and care teams as needed, ensuring continuity of care while promoting patient independence, safety, and overall well‑being. The RN – Float will deliver personalized clinical services in accordance with physician orders, agency policies, and regulatory guidelines, while adapting to varying schedules and coverage needs. Duties / Responsibilities Perform patient assessments and ongoing evaluations. Implement physician‑ordered plans of care and complete required documentation (OASIS). Provide in‑home nursing care for multiple patients per day. Educate patients and caregivers on medications, disease processes, and in‑home care. Coordinate patient services and supervise LPNs/HHAs as needed. Communicate changes in patient condition to physicians and care teams. Maintain compliance with regulations and agency policies. Physical Requirements Ability to sit, stand, and drive for extended periods while providing in‑home care. Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation. Regular use of a tablet for documentation, communication, and training. Current Registered Nurse license in the state of Indiana (required). Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred).
