Job Details: Schedule: Days shift, 8 hours per day, minimum of 40 hours per week Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; located in Indiana or willing to commute; valid state licensure in good standing; and a minimum of 1 year of relevant experience. Completes comprehensive evaluations and delineates goals focused on patient discharge within an established length of stay. Documents clinical information accurately and efficiently, including treatment plans, goal writing, and compliance with applicable guidelines.
