Registered Nurse (RN) – Home Health | DFW Area

IMPORTANT: This is a Dallas-Fort Worth, Texas-based home health position. Candidates must live in or within reasonable driving distance of the DFW area and be available to complete in-person patient visits throughout the assigned service area. This is not a fully remote position.

Job Summary

Boundless Home Health is seeking a Registered Nurse (RN) to join our growing team in the Dallas-Fort Worth area .

This is a home health field position that requires in-person visits to patients in their homes throughout DFW . While some administrative duties and documentation may be completed remotely, nurses must be available to travel locally to assigned patients.

We’re looking for an RN who is compassionate, dependable, detail-oriented, and comfortable working independently in the home health setting. You will be responsible for assessing patients, developing and managing individualized plans of care, monitoring changes in condition, and communicating with the clinical team to ensure quality patient care.

Responsibilities

• Complete in-person home health visits throughout the assigned DFW service area
• Assess patients and develop, implement, and update individualized plans of care
• Coordinate with physicians and other members of the care team
• Administer medications and monitor patients for side effects or reactions
• Monitor and document changes in patient condition
• Maintain accurate and timely clinical documentation
• Communicate patient updates and concerns to the appropriate supervisor
• Travel to patients’ homes as assigned

Qualifications

• Current Texas Registered Nurse (RN) license
• Previous RN experience preferred
• Home health experience preferred
• Strong assessment, documentation, and communication skills
• Excellent interpersonal and patient-care skills
• Valid driver’s license
• Reliable transportation and ability to travel throughout the assigned DFW service area
• Current auto insurance
• Current CPR/BLS certification
• Must reside in or within reasonable driving distance of the Dallas-Fort Worth Metroplex
• Must be available for in-person patient visits
