Advanced RN | Home Health Care Specialist
Join a dedicated healthcare team providing compassionate and expert nursing care in a regional home health setting within the Midwest. This role involves delivering direct patient care using the nursing process, demonstrating clinical expertise, and adhering to professional nursing ethics.
Essential Duties and Responsibilities
- Participate in agency councils, committees, or serve as a clinical coach, supporting professional development and quality care delivery. This may be waived for nurses currently enrolled in bachelor's or master's nursing programs.
- Administer medications and perform planned interventions safely and efficiently, following established policies and protocols.
- Communicate patient condition changes effectively to physicians, advocating for patient needs using structured communication methods like SBAR.
- Coordinate patient care collaboratively with healthcare team members and facilities to ensure seamless service delivery.
- Adapt positively to the dynamic healthcare environment, demonstrating professionalism and flexibility.
- Document patient care activities accurately at the point of care, including assessments, interventions, care plans, and physician communications using electronic medical records.
- Evaluate patient progress toward goals and update care plans accordingly.
- Develop age-appropriate, patient-centered care plans upon admission with active involvement of patients and families.
- Provide education tailored to patient and family needs, considering their educational level and cultural background.
- Engage in self-reflection to learn from experiences and improve practice.
- Apply critical thinking skills in assessing patient needs, planning care, and preparing for discharge.
- Utilize evidence-based practices and maintain up-to-date clinical knowledge and skills.
Education
Graduation from an accredited nursing school is required. A Bachelor of Science in Nursing is preferred.
Experience
At least one year of clinical nursing experience is required. Experience in home health nursing is preferred.
Certification and Licensure
Current RN license in the state or compact licensure is required unless a new graduate, who must obtain licensure within 90 days of graduation. American Heart Association Basic Life Support certification is mandatory.
Mental and Physical Requirements
This position requires considerable mental concentration and physical activity including lifting up to 35 pounds, frequent walking, standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching.
Working Conditions
Frequent exposure to communicable diseases, infectious materials, and potentially hostile individuals. Work involves handling sharps and biohazardous materials and exposure to varying environmental conditions across community settings.
