Integrated Services for Behavioral Health (ISBH) seeks an Engagement Care Coordinator to support youth enrolled in OhioRISE, coordinating care and guiding families through discharge planning and follow-up referrals. The role involves collaboration with families across home, school, and community settings, conducting crisis plans, and facilitating Child and Family team meetings until a warm handoff to ongoing care coordination.
