Hospice Nurse Practitioner

Position Summary Make a lasting impact by joining Providence at Home with Compassus , leaders in home-based hospice and palliative care. We’re seeking a compassionate and experienced Nurse Practitioner to provide essential support to patients and families across various care settings, including homes, long‑term care facilities, and hospitals. The Hospice and Palliative Care Nurse Practitioner functions independently, coordinates care, creates and modifies the plan of care, makes referrals, and collaborates with an interdisciplinary team. Position Specific Responsibilities Complies with organization‑wide policies and procedures. Complies with regulatory agencies’ patient care requirements. Creates a safe delivery environment and reports unsafe conditions. Conducts thorough medical, functional, and psychosocial assessments under physician supervision; performs diagnostic tests as defined by protocol. Provides medical interventions, initiates or alters drug therapies, and writes orders per physician protocols. Performs Face 2 Face, End of Life, and recertification assessment visits. Collaborates with the health team to provide continuity of care and share patient information. Orders drugs and supplies, keeps records as defined by protocol. Documents notes in the clinical medical record. Participates in interdisciplinary team meetings, recommends treatment courses, monitors ongoing care. Provides liaison and information to physicians, discharge planning, and other departments regarding quality of care issues. Recommends actions based on patient condition. Independently orders IV therapy, antibiotics, opioids, altering dosages per protocols. Furnishes medications per standardized procedures and formulary. Orders feedings and manages nasogastric or gastric tubes according to patient needs. Assists families with goals, values, and treatment decisions. Educates patients and families on medical condition and prognosis. Recommends referral to appropriate health facilities and community resources. Provides education to nursing staff and family members on patient needs. Maintains courteous relationships with public, patients, and personnel. Develops and implements evidence‑based educational practices. Performs other duties as required. Documents accurately and in a timely manner. Participates in provider on‑call rotation. Education and Experience Master’s Degree in Nursing. Two years of acute or transitional care setting experience. Certifications, Licenses, and Registrations Active and unencumbered RN license in the state of practice. National Provider BLS – American Heart Association (upon hire). Physical Demands and Work Environment The role requires regular standing, walking, manual dexterity, and the ability to lift up to 50 lbs. Visual acuity for close and distance vision, color and peripheral vision, depth perception, and focus adjustment are needed. Exposure to bodily fluids and infectious diseases is expected. Use of standard medical equipment and tools is essential. Compensation Salary range: $65.87‑77.88 per hour. Benefits Medical, Dental, Vision, 401k.
