Weekend Hospice RN Case Manager — 36h/wk, $15K Sign-On

A leading healthcare organization in Michigan is seeking a Hospice RN Case Manager to provide professional nursing care to patients. You will develop individualized care plans, coordinate services, and provide ongoing patient assessments. A minimum of one year in acute care nursing is required along with a current RN license.
