About the role

A Wisconsin Licensed Practical Nurse who is employed to identify, provide for and accept responsibility for the fulfillment of patient's total nursing needs. This includes all nurse practice acts such gathering data, medication management, and documentation.

Essential Job Functions

• Maintains and actively promotes effective communication with all individuals.
• Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
• Provide professional nursing services under the direction of a registered nurse in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and agency policy and procedure.
• Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Confer with the registered nurse regarding needed changes in the Plan of Treatment.
• Performs medical treatments and reports status to the registered nurse.
• Reports resident changes in condition promptly to the registered nurse.
• Prepare, submit and maintain documentation as required by the agency. Visit/shift notes documented on day services are rendered.
• Participates in following and updating plans of care.
• Promotes person centered care.
• Practice infection control mitigation at all times.
• Performs other duties, as assigned.

Requirements

Experience & Training

• Current Licensed Practical Nurse credentials and license in the State of Wisconsin.

Schedule

• Monday - Friday: 8:00 AM to 4:30 PM
• 0.8 FTE (32 hours a week), or 1.0 FTE (40 hours a week)

Location:

Hit the road without the extra costs—this role includes travel within a one-hour radius, and a company car is provided for employees working 0.6 FTE or more.

Pay

• $27.80 to $31.20 an hour, based on experience. Higher pay may be considered depending on qualifications and experience.
