Admissions Coordinator Full-Time | Skilled Nursing Facility Salary: Up to $68,000 per year Water's Edge Rehab & Nursing Center at Port Jefferson is seeking a detail-oriented and personable Admissions Coordinator to join our team. Position Overview The Admissions Coordinator is responsible for managing the intake and admission process, serving as a key point of contact for referral sources, hospitals, families, and internal departments. This individual plays a vital role in ensuring efficient transitions into the facility. Key Responsibilities Coordinate all aspects of the admissions process from initial referral through resident admission Communicate with hospitals, case managers, families, and referral sources Review clinical and financial documentation to ensure eligibility and appropriateness for admission Verify insurance coverage and obtain necessary authorizations Collaborate with interdisciplinary teams to facilitate smooth and timely admissions Maintain accurate records and census tracking Provide facility tours to prospective residents and families Assist in developing and maintaining strong relationships with referral sources Ensure compliance with all applicable regulations and facility policies Qualifications Prior experience in admissions, intake, or healthcare coordination preferred Experience in a skilled nursing facility or healthcare setting strongly preferred Knowledge of insurance verification, including Medicare, Medicaid, and managed care Strong communication, customer service, and organizational skills Ability to multitask and work efficiently in a fast-paced environment Proficiency with electronic medical records (EMR) systems; PCC experience a plus Compensation & Benefits Salary up to $68,000 annually (based on experience) Competitive benefits package including: Medical, Dental, and Vision Insurance Paid Time Off 401(k) Retirement Plan Employee perks and growth opportunities Water's Edge Rehab & Nursing Center at Port Jefferson is proud to be an Equal Opportunity Employer. #boost CareRite Centers
