The Enclave at Rye Rehabilitation & Nursing Center is seeking a compassionate and knowledgeable Registered Dietitian (RD) to join our interdisciplinary care team. If you are passionate about clinical nutrition and making a meaningful impact on the health and well-being of residents, we'd love to hear from you.

As a Registered Dietitian, you will play a vital role in assessing nutritional needs, developing individualized care plans, and ensuring residents receive the highest quality nutritional care in accordance with regulatory standards.
What You'll Do

• Complete comprehensive nutritional assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized nutrition care plans.
• Evaluate residents for nutritional risk and recommend appropriate dietary interventions.
• Collaborate with physicians, nursing staff, speech therapy, and the interdisciplinary team to optimize resident outcomes.
• Monitor weight trends, laboratory values, and nutritional status.
• Provide recommendations for therapeutic diets, nutritional supplements, and tube feeding regimens.
• Participate in care plan meetings and communicate with residents and families regarding nutritional needs.
• Ensure compliance with CMS, New York State Department of Health, and facility regulations.
• Maintain accurate and timely clinical documentation.
• Educate residents, families, and staff on nutrition-related topics and best practices.

Qualifications

• Current registration as a Registered Dietitian (RD/RDN) with the Commission on Dietetic Registration (CDR).
• Current New York State certification or eligibility to practice as a Dietitian.
• Previous experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or clinical healthcare setting preferred.
• Strong knowledge of clinical nutrition, therapeutic diets, and regulatory requirements.
• Excellent communication, organizational, and time management skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Passion for improving resident health and quality of life through nutrition.
