Outpatient Registered Nurse - RN - Dialysis

Responsibilities Provide and coordinate clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Delegate tasks to direct patient‑care staff (RNs, LVN/LPNs, Patient Care Technicians) as appropriate. Perform ongoing, systematic collection and analysis of patient data pre‑, during, and post‑hemodialysis, and document findings in the medical record; adjust treatment plans and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed. Assess, collaborate, and document patient/family basic learning needs to provide initial and ongoing education. Direct and collaborate with staff to ensure safe and effective delivery of hemodialysis therapy. Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness. Initiate or assist with emergency response measures. Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees. Ensure correct laboratory collection, processing, and shipping procedures, and reschedule missed or insufficient collections. Identify expected outcomes, document, and update nursing assessment and plan of care through collaboration with the Interdisciplinary Team. Ensure patient awareness of transplant and treatment modality options. Participate in education and quality improvement projects at the facility and area level as directed. Serve as a preceptor to new employees when assigned. Complete CAP requirements to maintain or advance the position. Perform all other duties assigned by the Supervisor. Physical Demands and Working Conditions Provides direct patient care that regularly involves heavy lifting and moving of patients, often up to 200lbs. and chemical and water solutions up to 30lbs. requires frequent, prolonged standing, bending, and ambulation assistance. The work environment is a healthcare facility with temperature control and moderate noise levels. Exposure to infectious diseases/materials is possible. The role may involve desk and computer work, interaction with patients, staff, and physicians, and occasional travel to training sites or other locations with similar conditions. Supervision Assisted by a Team Leader or designated Nurse in Charge who oversees Patient Care Technicians, LVN/LPNs, and RNs. Education and Licensure Graduate of an accredited School of Nursing. Current appropriate state licensure. Current or successful completion of CPR BLS certification. Must meet practice requirements in the state of employment. Experience and Required Skills Entry‑level RN with a minimum of two years of Nephrology Nursing experience. Minimum of three months experience in chronic/acute hemodialysis as an RN (for new employees completing FKC education and training).
