RN: Case Manager

Job Description Join a travel opportunity as an RN Care Manager at a hospital-based setting in York, Maine, coordinating concurrent utilization management activities and serving as the main bridge between the patient, the healthcare team, the organization, and payors during the hospital stay. You’ll lead and organize daily interdisciplinary care rounds/care conferences/care planning, conduct assessments of needs, identify the resources required to transition to the next setting, and secure those resources. This assignment is ideal for an experienced RN who has utilization review and discharge planning experience and enjoys practical, behind-the-scenes coordination that keeps care moving with purpose. Expect variable scheduling across the week with no call, plus weekend and holiday rotation as part of the team schedule. Core focus areas include partnering with community organizations to help secure durable medical equipment, arrange transportation, and complete health insurance authorizations, while participating in length of stay and aligning with hospital reimbursement methods and quality measures for the care management department.
