Job Title: Community Health Representative (CHR) Program Coordinator

Full - Time                            Non-Exempt                             Wage: TBD

General Definition: The CHR Program Coordinator serves as an advocate and facilitator for Native American citizens with their needs for access to healthcare services.  The highest duty of this position is effective and timely service delivery to Tribal members.

Supervision Received: Wellness Center Director

Supervision Exercised:     None

Essential Job Functions:

·         Provides health education information to Tribal members.

·         Coordinates and participates in tribal sponsored outreach events.

·         Facilitates and participates in health screenings for tribal citizens and the community.

·         Collaborates with the Early Childhood Learning Center (ECLC) providing needed services that nurture healthy physical and cognitive development of children.

·         Assists individuals in accessing healthcare services and resources .

·         Conduct community outreach and engagement activities and tribal sponsored community events .

·         Coordinates and attends health fairs and community events .

·         Offers culturally based support and advocacy for individuals navigating the healthcare system.

·         Document and track community health needs and outcomes.

·         Collaborate with healthcare providers and community organizations.

·         Develop and distribute educational materials and resources.

MINIMUM QUALIFICATIONS:

Education:

·         High School Diploma or GED required.

·         LPN required

·         RN preferred

·         Relevant work experience in a healthcare and/ or community setting required.

Knowledge, Skills, and Abilities:

·         Knowledge of local healthcare resources and services required

·         Experience in health education and promotion.

·         Culturally sensitive and knowledgeable.

·         Excellent communication and customer service skills.

·         Proficient in public speaking and presentations.

·         Able to communicate effectively with diverse populations.

·         Able to work independently or as part of a team as situation dictates.

·         Proficient in public speaking and presenting to an audience

·         Multi-Tasking and time management skills

·         Knowledge of Microsoft Office (Word, Excel, PowerPoint)

·         Strong problem solving and critical thinking skills

·         Respect for all Tribal Members, Tribal Officials, Supervisors, and Co-Workers

·         Excellent problem solving and conflict resolution

Licenses/Certifications/Other:

·         Valid driver’s license and reliable transportation

·         Valid CPR Instructor Certificate, or be willing to obtain one as a requirement.

·         Must be able to pass a background check

·         Must have verifiable personal and employment references

·         Must be able to complete pre-employment drug test with a negative result

Native American preference policy is as follows :  Legally enrolled Eastern Shawnee Tribal members shall be given preference if the tribal member’s experience, qualifications, training, and all other criteria including the job posting are equal.  Tribal Members or Citizens of other federally recognized Tribes shall have second preference following the same criteria, followed by any other applicants. (Resolution # 021011-R-03) Only Certified Degree of Indian Blood cards will be accepted as verification of Native American Heritage for other tribes.  Copy of CDIB card must be included with the application.
