Registered Nurse Home Health

Home Health Nurse

Provide comprehensive home health nursing care to patients in their residences, ensuring adherence to physician orders and agency policies. Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses to deliver quality patient care.

Responsibilities:

• Coordinate and deliver patient-specific nursing interventions in the home setting.
• Assess patient conditions, develop, and implement individualized care plans.
• Document all patient interactions and clinical activities accurately within required timeframes.
• Supervise and evaluate the performance of Home Health Aides and LPNs/VNs.
• Maintain compliance with state licensing, certification, and agency policies.
• Travel up to 75 miles for patient visits and attend periodic office meetings.

Qualifications:

• Valid RN license in good standing with current licensing requirements.
• Minimum of 1 year full-time adult home health or hospice experience.
• Proficient in Oasis documentation and HomeCare HomeBase EMR within the last 6 months.
• Reliable transportation, valid driver's license, and agency-required liability insurance.
• Ability to travel distances up to 75 miles and accept patient assignments regardless of location.
• Willingness to work flexible schedules, including possible weekend and on-call duties.
