Key Responsibilities Conduct detailed evaluations to assess patient physical therapy needs, including reviewing medical history and performing necessary assessments. Develop and clearly communicate individualized treatment plans, ensuring patients and their families are fully engaged in the goal-setting process. Deliver hands-on therapy sessions, adapting treatment protocols based on the patient's progress and treatment responsiveness. Maintain meticulous documentation and updates on patient care plans, utilizing electronic health record systems to ensure compliance and optimal patient tracking. Collaborate with multidisciplinary medical teams to address changes in patient conditions and refine treatment approaches accordingly. Participate in discharge planning and ensure continuity of care post-therapy by providing follow-up instructions and recommendations. Contribute actively to departmental goals and performance metrics, enhancing access to care initiatives and community outreach programs. Patient Population / Client Focus As a Physical Therapist at Forest Manor Rehabilitation Services, you will work with a diverse patient population, including those recovering from orthopedic surgeries, neurological conditions, and general rehabilitation needs. Understanding the unique challenges faced by these individuals, you will provide support and guidance that facilitates not only recovery but also empowerment and independence through physical therapy. Qualifications Bachelor's or Doctorate degree in Physical Therapy from an accredited program. Current state license to practice physical therapy in New York. Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association. Preferred Experience While prior work experience is not mandatory, candidates with previous internships or clinical rotations in outpatient settings are preferred. Experience with electronic documentation systems and familiarity with patient-centered care practices will be beneficial. Compensation and Benefits Forest Manor Rehabilitation Services offers an attractive salary range of USD 90,000 to 100,000 per year, commensurate with experience.
