How you will make an impact Providing education and counseling to patient/family regarding treatments and goals, potential safety risks and/or other issues within scope of practice. Evaluating physical and functional status, as well as cognitive, psychological, and perceptual ability to determine the patient’s capabilities to function (safely). Documenting all evaluations, treatment, and coordination of care contacts via appropriate facility policies. Independently managing own case load to maximize effectiveness of overall care plan. Duties & Responsibilities Physical Therapist is expected to conduct initial evaluations and accurately assess patient conditions to develop appropriate goals and treatment plans. Must be able to work an established schedule with flexibility according to daily clinic needs and complete adequate documentation through EMR software (we are currently using Athena). Therapist will be expected to meet the demands of the clinic's schedule, work effectively with support staff as appropriate, and perform other duties as required.
