Clinical Coordinator (RN)

Base pay range$90,000.00/yr - $100,000.00/yrThe primary function of this role is to support quality patient care at Obran Health's Home Healthcare Agencies in conjunction with the Care Team and Director of Nursing. You will leverage your experience in home health nursing to support field clinicians and direct home health services, ensuring that patients receive care in accordance with the agency's policies, procedures, and best practices. If additional information is needed, you will elevate the call to the Director of Nursing and ensure a prompt responseCollaborating with physicians, other healthcare providers, and community agencies to ensure coordinated careAnalyzing clinical data and identifying opportunities for improvementPromoting a positive and supportive work environment for clinical staffSupporting and educating new and existing staff to ensure quality patient care.Providing oversight and care coordination for designated caseload of patients.After hours/on call responsibilities to support the teamReviewing and signing Start of Care Orders, recertification orders, resumption of care orders, discharge orders and external ordersOther duties as assigned by supervisorRequirementsActive California RN BSN License1-2 years of experience with Home HealthCurrent CPR CertificationPreferred QualificationsKnowledge of intake, scheduling, authorizations, OASIS and ordersBenefitsLocation: This job is fully remote.
