Perform resident assessments and document findings in PCC electronic medical record with accuracy. Monitor vital signs and resident status; implement safety and falls-prevention protocols. Administer prescribed medications and treatments; report adverse events and respond to changes in condition. Assist with activities of daily living and routine nursing tasks, including personal care and mobility support. Collaborate with interdisciplinary team (RNs, CNAs, therapists) to coordinate care and ensure smooth shift handoffs. Maintain infection control standards, proper hand hygiene, and equipment sanitation.
