Speech Language Pathologist - Per Diem

We learn, grow, and achieve more – in our careers and in our communities. \n \n Job Description Summary: \n Competently interpret the patients’ history and baseline level of function and then to complete an assessment appropriate to the patient's areas of deficits. Based on problems identified, the speech language pathologist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Actively participates in department functions. There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.\n \n Responsibilities And Duties: Direct Patient Care 90-100%\n \nEvaluates and treats patients:\n \nAfter provider referral, evaluates patient communication, cognitive, functional status and/or swallowing, then develops, and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment.\n \nCarries out plan of care with reassessment and changes to plan of care as appropriate.\n \nPerforms and interprets instrumental exams and hearing screens as needed.\n \nTherapist is responsible for maintaining/meeting the productivity expectation of their department.\n \n \nPlans and helps coordinate discharge plan with multidisciplinary team:\n \nInstructs patient and care giver/family.\n \nRecommends diets/instrumentals required for discharge.\n \nProvides information to physicians, nurses, case managers, social workers, and other team members.\n \nInvolved in the multidisciplinary patient plan of care and discharge readiness plan.\n \nDocuments evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.\n \n \nIndirect Patient Care 1-10%\n \nDepartment Functions:\n \nSupervises patient assignments and duties of the rehab techs, as applicable.\n \nIs involved in department quality improvement through data collection and participation in team and staff meetings.\n \nAssumes duties delegated by management in the absence of department management.\n \nThere is an expectation that an employee in this position may be requested to work at care sites other than their home care site.\n \n \nEducation/Mentoring:\n \nTakes active role in orienting new staff members.\n \nServes as clinical instructor for affiliating SLP students and/or CFY placements.\n \nServes as an advocate for the discipline on the multidisciplinary team.\n \nRepresents department at problem-solving and committee meetings.\n \n \nThe major duties, responsibilities and essential functions listed above are not intended to be all-inclusive of the duties, responsibilities and essential functions to be performed by associates in this job.
