Occupational Therapist - Travel Contract

Make a meaningful difference as a travel Occupational Therapist in Logansport, IN. Provide skilled home health occupational therapy services to patients recovering from injury, illness, or disability. Perform evaluations, develop and implement individualized OT plans of care, complete OASIS visits and documentation, and collaborate with multidisciplinary teams to support patient independence and safety in the home setting.

Minimum Qualifications

• Master‘s or Doctorate in Occupational Therapy from an accredited educational program.
• Current, unrestricted Occupational Therapist license in Indiana required prior to start.
• Recent skilled home health experience required.
• Proficiency with HomeCare/HomeBase and OASIS documentation.
• Understanding of Medicare guidelines for home health services.
• Current driver‘s license, vehicle insurance, and dependable vehicle for travel between patient visits.
• BLS certification may be required per employer policy.

Preferred Qualifications

• Experience supervising COTA care per company policy and state regulation.
• Strong written and verbal communication skills and solid organizational skills.
• Ability to work independently and flexibility to meet patient scheduling needs.

Assignment Details

• Shift hours: Monday - Friday, 8:00 AM to 4:30 PM.
• Weekly hour guarantee: 36 hours.
• Mileage reimbursed between patient visits, from first patient to last patient.
• Coverage areas include counties such as White, Pulaski, Fulton, Cass, Carroll, Miami, and Howard (Tipton).
• May require participation in a weekend rotation and potential availability on observed holidays as needed to meet patient needs.

Deliver comprehensive home health occupational therapy that supports patient-centered goals, timely documentation, and interdisciplinary communication. Complete evaluations and OASIS visits including Discharge, Resumption of Care, and Add-On discipline visits. Participate in branch meetings and case conferences as specified by leadership. Schedule flexibility is required to meet patient care demands.
