Clinical - Patient Care Advocate

Job Title: Patient Care Advocate

Duration: 2 Months (Potential Extension and Conversion)

Location: Remote

Work Schedule: 8 hours per day M-F 8-5 PST, No overtime

PR: XXXXXXXXXXX to XXXXXXXXXXX/hr.

Day to Day Responsibilities:

• The team s purpose is to make outreach calls to members, educate them on the importance of preventative care and screenings.
• Address all member needs and create positive experiences.
• 8-hour workdays, no overtime.
• Complete member outreach calls to close care gaps for Medicaid.
• Educate members on the importance of preventative care.
• Focus on improved member experience.
• Work with other departments to resolve member concerns.
• Outreach call goals above 95 call per day.
• 14-16 appointments made/confirmed per day.

Years of experience required:

• Required: a bachelor s degree in healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration.
• Related health field or equivalent work experience required (a total of 4 years of experience required for the position); work experience should be in direct patient care, social work, quality improvement or health coaching preferably in a managed care environment.
• 2+ years of experience work experience should be in direct patient care, social work, quality improvement or health coaching, preferably in a managed care environment.

Disqualifiers :

• no remote experience

Additional qualities to look for:

• Member outreach experience. HEDIS knowledge, high volume outbound call

Skills:

• Previous experience in a role handling a high volume of outbound calls.
• The ability to work effectively in a remote environment while remaining self-motivated and driven to meet or exceed established goals and deliver measurable results. Self-discipline and time management.
• Ability to learn and adapt to new systems and company changes.
