Join our team at WCS Healthcare Partners as a Clinical Assessment RN, where you will play a crucial role in evaluating and coordinating care for consumers within Albany County. This hybrid position offers the flexibility of both in-home and remote work, allowing you to make a meaningful impact on patient care while maintaining work-life balance.
Responsibilities- Perform initial in-home pre-evaluation assessments of consumers.
- Review consumer medical documentation and health referral forms.
- Enter evaluation data into the UAS-NY online application.
- Code UAS assessments correctly based on guidelines.
- Document concerns, conflicting information, and outcomes.
- Emphasize continuity of care to reduce fragmentation.
- Discuss health care options and service vendor/waiver options with consumers.
- Initiate enrollment into managed care health plans.
- Stay updated on regulatory changes related to assessments.
- Improve care coordination with interdisciplinary team members.
- Review non-qualified assessments with the Quality Assurance Department.
- Collect quality review data and document case notes.
- Identify health promotion and illness prevention opportunities.
- Maintain knowledge of community resources and network services.
- Meet performance criteria and perform assigned duties.
- NYS RN Licensure required.
- 2 years of Homecare experience required.
- Must be able to travel throughout Albany County.
