Permanent Case Manager - Registered Nurse - Up to $50/hr

Full-time Home Health Case Manager – Registered Nurse provides comprehensive care and coordination for patients in their homes, ensuring continuity and quality of service. Based in Coeur d'Alene, Idaho, the role supports patients' recovery through individualized care plans and interdisciplinary collaboration. Key Responsibilities: Conduct comprehensive patient assessments and develop individualized care plans. Coordinate and manage patient care across the continuum, ensuring quality and efficiency. Provide skilled nursing care in accordance with physician orders and established protocols. Monitor patient progress and adjust care plans as necessary. Educate patients and families on disease management, medications, and care techniques. Collaborate with physicians, therapists, social workers, and other healthcare professionals. Maintain accurate and timely documentation in compliance with regulatory standards. Ensure compliance with all state, federal, and agency guidelines. Qualifications: Active Registered Nurse (RN) license in Idaho or compact license. Associate or Bachelor of Science in Nursing (ASN or BSN) required. Minimum of 1–2 years of nursing experience; home health or case management experience preferred. Current Basic Life Support (BLS) certification. Valid driver’s license and reliable transportation. Preferred Skills: Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Ability to work independently and manage time effectively.
