Behavioral Health Case Manager

***Duties and Responsibilities:***1. Ensure that patients receive appropriate quality care that is cost effective.2. Works with patients to assess their conditions and needs to develop personalized care plans.3. Conducts comprehensive assessments of clients' behavioral health needs4. Develop individualized care plans based on client assessments5. Coordinate and monitor implementation of care plans6. Provide case management services, including referrals and coordination of services7. Advocate for clients to ensure they receive appropriate and timely services8. Maintain thorough and accurate client records/documentation and scans hardcopy medical documents into EMR system.9. Work collaboratively with healthcare providers, social workers, and other professionals10. Provide crisis intervention and support when necessary11. Monitor client progress and adjust care plans as needed12. Facilitate group therapy sessions and support groups13. Educate clients and families about available resources and services14. Ensure compliance with all applicable laws, regulations, and standards15. Communicates with hospitals, pharmacies, health departments, clinics, insurance companies, government agencies, and health providers.16. Coordinate referrals between Specialty Providers.17. Performs other duties as assigned.1. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) certification2. Familiarity with community resources and support services3. Strong interpersonal and communication skills4. Ability to handle high-stress situations effectively5. Proficiency in electronic health records (EHR) systems6. Minimum of 2 years of experience in a healthcare or social services setting7. Knowledge of mental health and substance abuse disorders8. Excellent assessment and analytical skills9. Ability to develop and implement effective care plans10. Strong organizational and time-management abilities11. Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills12. Empathy and dedication to client advocacy13. Must be able to work independently.## Qualifications***Qualifications:***1. Master’s degree in social work, Psychology, or related field2. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) certification3. Current CPR Certification.4. Experience in case management or behavioral health settings.5. Certification in Case Management preferred.6. Willingness to undergo and pass background checks and drug testing. ***Typical Physical Demands:***Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for prolonged periods of time. Occasionally lifts and carries items weighing up to 50 pounds. Requires corrected vision and hearing to normal range.
