Responsibilities Provide outpatient rehabilitation services in an in‑home setting for older adults. Evaluate and treat patients in their most functional environment, such as private residences or independent living facilities. Organize and conduct occupational therapy programs to facilitate rehabilitation. Direct patient participation in selected tasks to restore, reinforce, and enhance performance. Facilitate learning of skills and functions essential for adaptation and productivity. Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel as required. Ensure adherence to department and patient treatment schedules, clinical and billing documentation, and work‑related meetings. Attend and contribute to patient care, staffing conferences, and other related meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Consult with rehabilitation team members, including community physicians, to select appropriate therapeutic regimens. Adapt programs or plans of care according to individual patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order, maintain, identify, and report supplies and equipment needs. Enforce patient‑first service: ensure that patients and families receive the highest quality of service in a caring and compassionate atmosphere. Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures. Develop treatment goals and methods in collaboration with patients and caregivers. Implement occupational therapy treatment plans and complete related documentation. Provide therapeutic techniques in areas such as cognitive skills, muscle strength, coordination, endurance, mobility, perceptual abilities, sensory awareness, sitting and standing tolerance, balance, activities of daily living, joint protection, work simplification, orientation, physical agent modalities, low vision, falls risk management, medication management, health and wellness, community integration. Design, fabricate, fit, and apply adaptive devices, orthotics, and prosthetics, and train patients in their use. Select appropriate activities suited to an individual’s physical and mental capacity, intelligence level, and interest. Instruct patients in skills and techniques required for functional activities and evaluate progress, attitudes, and behavior. Perform routine re‑examinations as needed to modify and progress the plan of treatment. Perform other related duties as required. Qualifications Bachelor’s degree in an occupational therapy curriculum accredited by ACOTE standards. Initial registration from the National Board for Certification in Occupational Therapy. Valid and unencumbered occupational therapy license in the state of practice. Valid driver’s license or reliable, independent transportation to and from patient residences.
