Join Our TeamJoin a forward-thinking, value-based care organization focused on delivering exceptional preventive primary care for adults 65+. This model empowers physicians to build meaningful patient relationships, spend more time with each individual, and practice medicine the way it was meant to be practiced–without RVU pressure.We welcome physicians with Value-Based Care (VBC) experience as well as those eager to learn.Compensation & IncentivesBase Salary: $235,000-$270,000 (commensurate with experience & location)Commencement Bonus: up to $75,000 (varies by clinic)Annual Performance Bonus: 20% of base salaryFull Benefits on Day 1Malpractice: Occurrence-based coverageRelocation Assistance: Available for moves >50 milesEarly Sign-On Bonus OpportunitiesTotal 1st Year Compensation: $300K+ (higher with experience & performance)Role HighlightsPatient Population: Adults 65+ (majority Medicare Advantage, chronic disease & preventive care focus)Census: 12-15 patients per dayVisit Length: 20-40 minutes per visitIntegrated Care Team: NP/PAMedical AssistantsSocial WorkersLab & Pharmacy supportReferral coordinationImpact-Driven Environment: Opportunity to make a meaningful difference in senior health outcomesTraining & Professional Development3-Week Physician Learning Journey Comprehensive onboarding that includes: Value-Based Care trainingDocumentation best practicesEMR optimization (Athena–currently in transition)Integrated care model workflowRequirementsBoard Certified in Internal Medicine or Family MedicineClear and active medical licenseDEAGeriatric experience preferredHEDIS experience a plusValue-Based Care experience preferred–but not required; training providedNO RESIDENTS, NO NEW GRADS
