Job Summary:

The Home Health Registered Nurse (RN) with OASIS experience is responsible for providing skilled nursing care and assessments to patients in their homes. This role involves conducting patient evaluations using OASIS standards, managing patient care plans, administering treatments, and collaborating with interdisciplinary teams to deliver high-quality outpatient care. The RN must be adaptable to schedule changes and capable of traveling between patient locations, ensuring effective communication and care coordination. Responsibilities:

• Perform patient assessments and complete OASIS documentation accurately.
• Develop and implement individualized care plans based on patient needs.
• Administer medications and treatments as prescribed.
• Coordinate care with interdisciplinary teams including physicians, therapists, and social workers.
• Provide education and support to patients and their families regarding care and treatment plans.
• Travel to patient homes to deliver nursing care and conduct follow-up visits.
• Maintain accurate and timely electronic medical records using HealthWyse (Careficient) EMR system.
• Manage and adapt to changes in patient schedules and care requirements.
• Participate in on-call rotation for emergency situations as needed.

Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of 2 years of home health and hospice nursing experience.
• Proficient in OASIS (Outcome and Assessment Information Set) assessments and documentation.
• Current Basic Life Support (BLS) certification.
• Valid driver's license with a good driving record and reliable automobile for patient visits.
• Experience with electronic medical record systems, preferably HealthWyse (Careficient).
• Strong clinical assessment, medication administration, and care planning skills.
• Excellent communication and interpersonal skills for patient and team collaboration.

Preferred Skills & Certifications:

• Experience working in outpatient care settings.
• Additional certifications related to hospice or specialty nursing care.
• Familiarity with interdisciplinary team collaboration in home health environments.

Special Considerations:

• Some weekend coverage is required.
• On-call duties may be scheduled but only on an emergency basis.
• Must be able to travel frequently between patient homes.
• Ability to manage and adjust to changes in daily schedules.

Scheduling:

• Monday through Friday, 8:00 AM to 5:00 PM day shifts.
• Some weekend coverage required as part of the schedule.
• Emergency on-call availability on a limited basis.
