Primary Care Physician - Virtual Primary Care Practice

Jefferson Health | Remote/Virtual | Full-Time Jefferson Health is seeking a board-certified or board-eligible Family Medicine or Internal Medicine physician to join our nationally recognized Virtual Primary Care (VPC) Practice . This is an opportunity to build meaningful, longitudinal relationships with patients while delivering comprehensive primary care in an innovative, all-virtual environment. While the position is remote, preference is given to physicians residing in Pennsylvania, New Jersey, or Delaware. Why Join Jefferson Virtual Primary Care? Position Highlights Full-time, remote primary care practice Longitudinal care for an established panel of adult patients ages 18+ Comprehensive preventive, acute, chronic, and complex care Integrated behavioral health, social services, lifestyle medicine, and population health support Collaborative, interdisciplinary care model Telehealth-based transitions-of-care and supplemental access visits Epic electronic health record Shared telephonic call responsibilities Opportunities to participate in research and education based on individual interests Comprehensive Jefferson benefits and retirement programs Academic and professional resources through Thomas Jefferson University Your Practice Physicians provide comprehensive, relationship-based primary care through telehealth, including preventive care, chronic disease management, acute care, diagnostic evaluation, medication management, and coordination with specialists and higher levels of care when appropriate. You will work closely with patients and Jefferson's broader care team to address care gaps, improve access and outcomes, and support value-based care and quality initiatives.
