Registered Nurse LTC/SNF

Job Summary:

Responsibilities:

• 
  Assess residents health conditions and develop individualized care plans.
  
  
• 
  Administer medications and treatments in accordance with physician orders and facility protocols.
  
  
• 
  Monitor residents responses to treatments and update care plans accordingly.
  
  
• 
  Communicate effectively with residents, families, and interdisciplinary team members.
  
  
• 
  Ensure compliance with all safety, health, and regulatory standards.
  
  

Qualifications:

• 
  Valid state registered nurse license required.
  
  
• 
  Proven experience in LTC/SNF environments preferred.
  
  
• 
  Ability to assess and prioritize resident needs effectively.
  
  
• 
  Strong documentation and communication skills.
  
  
• 
  Must adhere to facility policies including identification and badge requirements.
