RN Care Coordinator - Discharge & Care Continuity Lead

Wellstar Sylvan Grove is seeking a Registered Nurse Care Coordinator to assess transitional care needs, coordinate care across the continuum, and engage with patient and family to assure care needs are met. The CC RN plans effectively to meet the patient's needs, manage length of stay and promote efficient utilization of resources, collaborating with the care team to discharge patients timely.
