Position Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Bridgeview, Illinois. Responsibilities Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager. Provide high-quality patient care in the communities of Justice, Bellwood, and surrounding areas. Qualifications RN license in the state where you work. Graduate from an approved school of practical nursing. One year of RN experience and the clinical competence to deliver quality patient care. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations. A commitment to consistently meet critical deadlines for charting. The skills needed to self‑manage your time and schedule. Demonstrated experience with tablets, mobile phones and EMR software.
