Registered Nurse (RN) Royal Middletown Nursing and Rehab – AuthentiCare Health Centers Job Summary Royal, part of AuthentiCare Health Centers, is seeking a compassionate, dependable, and dedicated Registered Nurse (RN) to join our skilled nursing team. As an RN, you will provide high-quality, resident-centered nursing care while supporting residents' health, safety, comfort, and overall well-being. You will assess residents' conditions, coordinate care plans, administer medications and treatments, and collaborate with healthcare professionals to ensure quality care in a skilled nursing environment. Administer medications and treatments as prescribed and in accordance with applicable regulations and facility policies. Monitor residents' vital signs, symptoms, and progress, responding promptly to changes in condition. Coordinate resident care with physicians, nursing staff, and other healthcare professionals. Educate residents and their families about health conditions, medications, treatment plans, and discharge instructions, as appropriate. Maintain accurate and timely clinical documentation in the electronic medical record. Participate in care planning and case management to promote continuity of care. Follow infection prevention, resident safety, and quality assurance procedures. Maintain resident confidentiality in accordance with HIPAA requirements. Support residents receiving skilled nursing, long-term care, subacute care, and memory care services. Assist with rehabilitation-related care, pain management, and other specialized services as appropriate to the resident's needs and the RN's scope of practice. Provide guidance and support to nursing team members as needed. Respond to emergencies and follow established facility protocols. Qualifications Current, active Registered Nurse (RN) license in the state of employment. Strong knowledge of nursing principles, clinical assessments, medication administration, and resident care practices. Excellent clinical judgment and ability to recognize and respond to changes in residents' conditions. Strong communication and interpersonal skills when working with residents, families, and healthcare professionals. Ability to maintain accurate clinical records and protect confidential information. Strong organizational skills and the ability to prioritize responsibilities in a fast-paced healthcare environment. Commitment to resident dignity, safety, and quality care. Preferred Qualifications Experience in skilled nursing, long-term care, subacute care, or memory care. Familiarity with PointClickCare (PCC) or other electronic medical record systems. Knowledge of Minimum Data Set (MDS) assessments and long-term care documentation. Experience with care planning, case management, or supervising nursing team members.
