Responsibilities: Provide clinical triage and nursing assessment via phone and telehealth platforms to determine acuity, prioritize care needs, and guide appropriate next steps Utilize evidence-based triage protocols to assess symptoms, provide patient education, and escalate care when necessary Collaborate with interdisciplinary teams, caregivers, and providers to ensure continuity of care and patient safety Accurately document assessments, interventions, and outcomes in EHR system KanTime while maintaining HIPAA compliance Interested in hearing more?
