Registered Nurse - Home Health

Schedule

8-hour shifts.

Assignment Details

Contract length: 6 weeks. Guaranteed hours: 40 hours/week.

Requirements

• Current state license as a Registered Nurse.
• 1-2 years of experience as a Registered Nurse in a clinical care setting.
• 2+ years of home health experience.
• Homecare Homebase software experience.
• OASIS experience.
• Current CPR certification.
• CHPN certification (preferred).
• Must have reliable transportation and insurance.

Responsibilities

• Assess home health and/or hospice patients in stable and emergent situations.
• Implement, develop, document, and evaluate the Plan of Care (POC).
• Educate patients and families regarding care, diet, medications, and home safety.
• Supervise and provide clinical direction to home health aides or similar personnel.
• Monitor assigned cases for compliance with third-party payer requirements.
• Perform on-call responsibilities as assigned.

Compliance / Notes

All required certifications must be current and maintained throughout employment.
