As a Social Worker with WesleyLife At Home, you will:
- Explain WesleyLife Hospice and Palliative services to patients and families.
- Visit patient homes (wherever they call home) to assess the psychosocial status, safety concerns, and bereavement risk factors of patients and families/caregivers.
- Communicate findings to patient care teams to update care plans.
- Carry out social evaluations and plan interventions.
- Counsel patients and families/caregivers regarding stress and other identified coping difficulties.
- Provide information and referral services for patients and families/caregivers regarding financial, practical, and environmental needs.
- Serve as liaison with patients and families/caregivers and care providers or community agencies.
- Participate in caregiver, placement, and discharge planning when needed, including discharge to a higher level of care.
- LISW preferred, LMSW highly encourage to apply.
