This role involves evaluation, treatment planning, interdisciplinary collaboration, and documentation compliance. Key Responsibilities Evaluate residents and develop individualized treatment plans Provide skilled occupational therapy services per physician orders Document evaluations, treatments, and progress notes in compliance with regulations Collaborate with physicians, nursing, and interdisciplinary team members Educate residents, families, and caregivers on treatment plans and home programs Participate in care planning, discharge planning, and team meetings Conduct staff training and in-service education Maintain compliance with state practice acts and facility policies Qualifications Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred) Active Occupational Therapist license in South Carolina (in good standing) Experience in skilled nursing or rehabilitation setting preferred Familiarity with PointClickCare and Casamba preferred Strong clinical, documentation, and communication skills We are an equal opportunity employer.
