The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient‑centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home‑based therapy. What You’ll Do Provide skilled physical therapy treatments in patients’ homes under the direction of a licensed Physical Therapist Carry out individualized treatment plans focused on mobility, strength, balance, and functional independence Assist patients with therapeutic exercises, gait training, transfers, and mobility training Educate patients and caregivers on safe mobility techniques, home exercise programs, and fall prevention strategies Observe, document, and report patient responses and progress to the supervising Physical Therapist Communicate effectively with physicians, nurses, and other members of the interdisciplinary care team Maintain accurate, timely, and compliant clinical documentation Ensure compliance with all Medicare, CMS, and home health regulations.
