Job Description

The RN Case Manager in acute care coordinates and advocates for hospitalized patients’ care, ensuring safe, timely, and cost-effective transitions from the hospital to appropriate post-acute settings.

Key Roles & Duties
Conduct intake assessments with patients and families to evaluate medical, functional, and psychosocial needs.

Develop individualized care/discharge plans that account for physical, financial, and insurance constraints.

Coordinate discharge planning, including arranging skilled nursing facility (SNF) placement, home health services, or durable medical equipment.

Collaborate with healthcare teams (providers, social workers, insurance) to secure approvals, discuss progress, and adjust treatment protocols.

Verify insurance coverage and devise solutions/referrals for patients who cannot afford treatment.

• Years of experience REQ: 2-year acute medical Care Manager experience performing Care Coordination
• First-timers accepted: No - requires travel experience, HARDSTOP
• Weekend REQ: Weekend Rotation
• CA state license: Yes
• Pending License accepted: Yes, but must be licensed upon start date
• RTO Restrictions: No more than 7 days
