Shift Type: 8-hour day shifts, Monday-Friday or Tuesday-Saturday options, every other weekend required

Hours per Week: 40

Candidate Pay Rate: $2900- $3000/ Week

Roles & Responsibilities

• Provide skilled nursing care to patients in their homes
• Conduct comprehensive assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Educate patients and families on disease management and self-care
• Coordinate with interdisciplinary team for holistic patient care
• Document patient progress and communicate changes to physicians

Qualifications

• Active RN license in California
• Minimum 2 years of home health experience
• Current BLS certification
• Valid driver's license and auto insurance
• Experience with HomeCare HomeBase (HCHB) preferred
• Ability to work independently and manage a caseload

Knowledge & Skills

• Expertise in home health nursing and patient education
• Knowledge of CHF, COPD, wound care, and IV management
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Comfortable with EMR systems, especially Cerner
